Notes:

Similar to other releases from 1964, however this version has a 2 printed on back jacket lower right corner to the left of RIAA logo. There is also a 2 etched in the inner grooves of side 2 before the matrix number, there is no etched number on side 1. 

T 2110 on jacket, T-2110 on labels.
